When it is available information on where to get vaccinated in your community will be posted at pandemic.wisconsin.gov and on your local health department website.

Vaccine Pre-Registration
Preregistration is closed as of Friday, September 25, 2009 at 10:00am. All facilities that are accepted from preregistration will receive an email with instructions on how to register. If you did not preregister but wish to receive the H1N1 vaccine AND meet the following criteria:
- You must be a hospital, clinic, local health department, residential care facility, student health service, pharmacy, occupational health service, mass immunizer AND
- You can maintain any H1N1 vaccine received in cold storage AND
- You can utilize the minimum shipment of 100 doses of H1N1 vaccine for both your staff and patients/residents
Please send an email to dhsh1n1vaccine@wisconsin.gov indicating the type of facility, e.g. hospital, clinic, etc. your county and the number of doses of seasonal influenza vaccine that you administered last year (2008-2009) for staff and patients and/or residents. If approved, you will then receive an email with a link on how to register to receive the H1N1 vaccine.

Doses Administered Reporting
Providers are strongly encouraged to record immunizations administered to individuals in the Wisconsin Immunization Registry (WIR), or RECIN (if applicable). This creates a permanent record accessible to all healthcare providers authorized to access the WIR. - Option 1: Direct Data Entry Into WIR - Individual patient immunizations are entered into the WIR, either through typical WIR access or the WIR Mass Vaccination Option.
- Option 2: Electronic Bulk Data Exchange - This option is for those entities already routinely providing data to the WIR through data exchange, or for those using the RECIN system. For more information about this option, please contact the WIR Help Desk at (608)266-9691.
- Option 3: Aggregate Reporting Survey - Collection of data and reporting aggregate totals of individuals vaccinated by age group and dose number via the electronic survey once a week.
